Been a while since I checked it out, but I think it's essentially the gnu-utils package that is added, replacing the busybox stuff with a "proper" version. Pretty it would be the same if you pre-downloaded gnu-utils.dsl and installed it after running a "standard" install - well, last time I checked that's all it was. Supposed to be better. From what I can tell, the busybox stuff is kinda a half-arsed replacement for a heap of tools, kinda does the job, but if you want to do anything reasonably serious you replace it with the standard set. Which I'm guessing would be the case if you intend doing a HD install. About 9MB I think.
